Visiting Info:
When can people be allowed to visit? Our Visiting Hours are from 10:00 AM to 7:30 PM, seven days a week.
Do you allow privacy with the resident when family members are visiting? Yes, we value and encourage family members to come in and spend as much personal, one on one time with their loved ones. It helps the residents to feel connected and loved.
Do you allow resident on outings, such as to a park, a restaurant, or to a family function? Yes, outings also need to be scheduled around doctor’s visits or tests that are scheduled for the resident. Residents need to be signed out by the responsible party at the nurse’s station.

Activities:
Are there meaningful activities for groups and individuals? Yes, each day at Gracelen Terrace is filled with all types of activities to not only entertain but to also educate and inspire.
Are there therapeutic activities, like music, pet, or plant therapy? Yes, we have many unique musical groups who come in through out the month, special groups that bring in not only domestic animals but exotic animals, we also have various horticulture activities.
Are there opportunities for your loved one to socialize? Yes, we offer a wide variety of social activities, chat-time, sing-along's, poetry corner, church services, movies and more.

Philosophy of Care:
Does the facility focus on the needs of the individual resident? Yes, Gracelen Terrace goals is to not only provide superior care for each individual but to provide an enhanced level of quality care for each resident, especially people with challenging behaviors.
Are there regular care planning meetings regarding my loved one? Yes, these meeting occur on a quarterly)
Will these meetings include myself and other family members as well? Yes, we notify the family 30 days before the meeting in writing. If you have scheduling conflict will try to accommodate your schedule, a phone conference is available as well.

Nursing
What can I expect from your nursing staff?
We have licensed nurses on duty 24 hours a day, seven days a week. Our nurses have to meet standards set by the State of Oregon's Board of Registered Nurses for licensure. The charge nurses supervises a staff of certified assistants (CNA's) who help residents with bathing, dressing, and meals. In addition, the CNA staff take daily vital sings; i.e., blood pressure, temperature, and pulse and report these to the charge nurse. Family members may also call the charge nurse at any time for a status report on a resident's medical condition. We are here to assist with the family's concerns as well as the residents. Each resident has a resident care manager (RCM) assigned to them to ensure all the care needs are met. Please feel free to ask for them as well.

Special Medical Needs:
What if my loved one needs a consultation for other medical services? We contract with many ancillary service providers including (but not limited to) the following: Audiology, Psychiatry, Psychology, Dentistry, Orthotics & Prosthetics, Podiatry, X-ray services, Optometry, Opthamology, Dermatology, Respiratory Therapy.

Medical Care:
Can I continue to use members of my loved one's healthcare team? Yes, or you can use our house physcian DR. McGowan, we also have a geriatric Nurse Practitioner on staff who specializes in behavior management.
Is there a doctor always available or on call in case of emergencies? Yes
How often does a doctor visit? Our physician is required to see residents shortly after admission, and every 2 months after that. DR. McGowan visits twice a month and ias available if a resident has any needs.
Can I meet the doctor? Yes, let the RCM (Resident Care Manager) know you would like to speak with him and they can help facilatate your request.

Dimentia-Friendly:
If my loved one has dimentia, is the staff specially trained to care for someone with this condition or other forms of dementia? Yes, Gracelen Terrace staff specializes in dimentia and residents with challenging behaviors. Our staff is required to attend our continuing education programs twice a month.
Are dimentia residents able to wander safely indoors and outside? Yes, residents are allowed to wander freely throughout the facility. Our exterior park-like setting is securely fenced and we encourage residents to enjoy the outdoors at liberty in a safe environment.

Is there public transportation available nearby the facility? Yes, we are between SE Powell Blvd and Hogate Blvd that offers Tri-Met bus service every 15 minutes from 5:30 AM - 11:30 P.M. Facility is located at 10948 SE Boise, Portland OR 97266, Phone: 503.760.1727.

What type of Safety devices are used:
Does your facility utilize smoke detectors, grab rails, bed rails, etc.? We met all state and federal regulations for fire safety and are inspected annually by the state and city firemarshalls.

Individualized Care:
Is consideration given to individual cultural, religious or spiritual needs? Yes, We have weekly religious services on Tuesday, Communion on Wednesdays and non denomiation services every other Sunday.
Are other languages spoken? Yes, we contract with a interpreter service for all languages.

Who ensures regulations?
Yes, State of Oregon's CCMU (Client Care Monitoring Unit), we also have ombudsman who visits the facility.
